The program is designed to help you understand your stage of CKD and what it means to your life.
Stepping On Program
The Stepping On program offers information on how to reduce falls giving you the confidence to undertake everyday activities safely. This program is a community-based workshop for both men and women, offered once a week for seven weeks using adult education principles that build confidence, knowledge, and skills to reduce falls.
EnhanceFitness is a group exercise class that improves your balance, flexibility, bone density, endurance, coordination, and mental sharpness while decreasing your risk of falling.

Freedom From Smoking
Learn how to overcome your tobacco addiction so you can start enjoying the benefits of better health. Our Freedom From Smoking classes are small group settings that provide a fun, interactive learning environment. Participant workbook and other informative materials are provided.
Powerful Tools for caregivers
This evidence-based workshop is designed to have positive impact on the health of a diverse group of caregivers, including adult children of aging parents, spouses/partners, caregivers at differing stages in their caregiving roles, living situations, financial, and educational backgrounds.
National Diabetes Prevention Program
It’s time to take charge of your health. Prevent Type 2 diabetes with the National Diabetes Prevention Program. This CDC-approved 12 month program is given in 16 weekly sessions, followed by monthly maintenance sessions. Our lifestyle coaches are typically Registered Dietitians or Registered Nurses and will help you develop healthy eating habits, increase your physical activity, and help keep you motivated to make healthy changes.
